Service

Fee Structure

  • A one-on-one patient interview to gather health conditions, current &  past prescribed medications, over-the-counter medications & supplements, and the patient’s beliefs about taking medications
  • A complete medication regimen review to ensure safe, effective and cost-justified medication therapy
  • Identify, prevent and resolve medication-related problems
  • Patient and care giver education on health conditions and prescribed & over-the-counter medications
  • Communicate with primary care providers as needed to offer suggestions for drug therapy
  • Written Care Plan  addressing current medication issues
  • Periodic follow-up care to ensure optimal drug therapy outcomes and continue evaluation with added or changed medications
$125.00 per hour, billed in 15 minute increments.CMRs take between 1- 4 hours, depending on the number of medications and health conditions.Below is the estimated time a pharmacist will spend to complete a CMR based on the complexity of the medication regimen:

 
1-5 Medications 1 hour
6-15 Medications 2-3 hours
16-25 Medications 3-4 hours
>25 Medications 4-5 hours

We have experience working closely with government agencies, including numerous successful surveys with The Joint Commission.Standard Services

  • Medication regimen reviews as required by CMS (or Department of Health) for each individual resident. Report included.
  • Evaluation of all medication practices including the following:
        -    Appropriate storage
        -    Safe administration
        -    Appropriate monitoring & documentation
        -    Report of findings & recommendations for improvement
  • Medication destruction and/or redistribution to the state approved Unused Prescription Drug Program, as requested and appropriate
  • Staff education on medications and common health conditions in the senior population

 Additional Services (negotiated)

  • Quality Assurance Committee attendance and participation
  • Survey attendance and involvement
  • Comprehensive Medication Reviews for residents, by appointment

Service

Fee Structure

Spend more time accepting new patients and billing at a level 4 or 5 reimbursements (CPT codes 99214 and 99215). Leave the medication management of chronic conditions to us!  Our collaboration with you as a physician is through established treatment protocols that you approve, allowing you time to spend with your more complicated patients.   Medication Therapy Consults

  • Comprehensive medication evaluation looking for drug-drug interactions, drug-disease interactions, drug-food interactions, adverse effects, appropriate therapeutic outcomes, and cost-containment
  • Evaluate and improve patient compliance with medication regimens
  • Medication reconciliation between differing physicians and levels of care
  • Evaluate herbal, vitamin, mineral and nutritional supplements
  • Assistance with complex medication regimens
  • Guidance with discontinuing medication to avoid adverse events

$125.00 per hour, billed in 15 minute increments.

Fee Structure

  • Filling Pill Boxes                                          $350.00
    This is a weekly service in which we provide accurate pill placement for each day and appropriate time to take medications
  • Obtaining Refills                                           $50.00
    We call the pharmacy or doctor to request refills, though family members must pick them up or have them delivered
  • Medication reminders                                $100.00
    We provide automated telephone reminder calls to clients as many times as needed to take their medications
  • Individualized Reports                                $100.00
    We send reports to loved ones via email weekly or monthly on compliance and any noted medication issues that should be brought to the physician’s attention
$500.00 per person each month for all services. Individual services as per listed price.
